Bleomycin can cause very specific hyperpigmentation, which occurs mainly on the trunk and the proximal extremities. The lesions are mostly asymptomatic, although in some cases may itch. The exact mechanism of flagellate pigmentation is still unclear. The lesions are harmless and resolve spontaneously after discontinuation of bleomycin.
